Initial report may be in various forms, but a written report shall be completed as soon as possible. 若適用，書面報(bào)告應(yīng)當(dāng)包括下列內(nèi)容，并將相關(guān)文件的副本與照片附在書面報(bào)告上： Where applicable, the written report shall include the following contents, and the duplicate of the relevant document and the photograph shall be attached to the written report: （一）事故或者事故征候發(fā)生日期； (1)Date when the accident or incident occurred; （二）事故或者事故征候發(fā)生的地點(diǎn)、航班號(hào)和飛行日期； (2)Place where the accident or incident occurred, flight number, and flight date; （三）有關(guān)貨物的描述及貨運(yùn)單、郵袋、行李標(biāo)簽和機(jī)票等的號(hào)碼； (3)Descriptions of the goods concerned and the number of air waybill, mail bags, baggage tags and tickets; （四）已知的運(yùn)輸專用名稱（包括技術(shù)名稱）和聯(lián)合國編號(hào)； (4)Known proper shipping name (including the technical name) and UN number; （五）類別或者項(xiàng)別以及次要危險(xiǎn)性； (5)Class or division and the subsidiary risk; （六）包裝的類型和包裝的規(guī)格標(biāo)記； (6)Packaging type and packaging specification marking; （七）涉及數(shù)量； (7)Quantity involved; （八）托運(yùn)人或者旅客的姓名和地址； (8)Name and address of the shipper or passenger; （九）事故或者事故征候的其他詳細(xì)情況； (9)Other detailed information about the accident or incident; （十）事故或者事故征候的可疑原因； (10)The suspicious cause of the accident or incident; （十一）采取的措施； (11)Measures to be taken; （十二）書面報(bào)告之前的其他報(bào)告情況； (12)Other information reported before the written report; （十三）報(bào)告人的姓名、職務(wù)、地址和聯(lián)系電話。 (13)The name, title, address and telephone number of the person who reports. 第九十三條 Article 93 當(dāng)在貨物或者郵件中發(fā)現(xiàn)未申報(bào)或者錯(cuò)誤申報(bào)的危險(xiǎn)品時(shí)，經(jīng)營人應(yīng)當(dāng)向經(jīng)營人所在國和事件發(fā)生地所在國有關(guān)當(dāng)局報(bào)告。 The operator shall report to the appropriate authorities of the State of the Operator and the State in which this occurred any occasion when undeclared or misdeclared dangerous goods are discovered in cargo or mail.當(dāng)在旅客行李中發(fā)現(xiàn)根據(jù)《技術(shù)細(xì)則》要求不允許攜帶的危險(xiǎn)品時(shí)，經(jīng)營人應(yīng)當(dāng)向事件發(fā)生地所在國有關(guān)當(dāng)局報(bào)告。 |
